How the plan works
|
The PSEA Health & Welfare Trust provides a death
benefit for you at no cost to help protect your family
from the financial burden that death could bring if
you die for any reason while covered by the plan
|

Eligibility
|
- Under age 70 and eligible for benefits under the
PSEA Health and Welfare Trust (see "Starting Out in
the Program" in your Benefit Booklet).
- Trustees and employees of the PSEA if under age
70
Note: No coverage is provided if you are age 70
or older, and no dependents are covered regardless of
age

Coverage

If you die while covered by this plan, your beneficiary
will receive $10,000
|

Beneficiary
|
You may designate a beneficiary in writing and file
it with the PSEA Health & Welfare Trust
